Due to our continued commitment in supporting people suffering with substance misuse problems, Change Grow Live are looking for a Homeless Recovery Coordinator to join our multidisciplinary team in

Tower Hamlets RESET Treatment Service. The team supports service users with drug issues from point of entry into the service and through their treatment/recovery journey.

As part of the Ministry of Housing, Communities & Local Government funding package for 43 areas with the highest numbers of people sleeping rough, Change Grow Live services have been involved in developing funding proposals along with other stakeholders.


We are pleased to announce that we have been successful in securing funding and are now looking to recruit staff to join dedicated multi-disciplinary teams across the country to support rough sleepers with harm reduction and to engagement in treatment and will sit within drug and alcohol treatment services.


Change Grow Live services and staff will provide wraparound support to rough sleepers and will work directly with emergency accommodation to support the staff with this group of people.

We are looking for enthusiastic individuals and friendly faces to help support people in leading healthier lives.


About the role:


  • Establish effective communication and referral pathways those individuals who are
homeless and in contact with other health and care agencies.

  • Attend relevant multidisciplinary team meetings supporting links to health and social
care services, peer support networks, community assets and mutual aid.

  • Work closely with the local authority, Health & Social care sector, partners and any
provision linked to Homelessness and improve joint working.

  • Liaise with other services involved in the care and support of an individual to ensure
appropriate and secure transfer of all care needs.

  • Coordinate the transfer of any accommodated out of area individual. This may include
referral to and liaison with their new drug and alcohol service and pharmacy and
provision of bridging prescription until an appointment and prescription is provided.

  • Increase and enhance partnership working with primary care, mental health services,
housing and other voluntary organisations who are supporting rough sleepers.

  • Ensure there are culturally appropriate interventions for hard to reach individuals from the Eastern European and Sub-Saharan African rough sleeping communities.
  • Utilise existing/new networks and pathways to broker support from strategic partnerships
and other providers.

  • Ensure there is continuity of care for those moving out of the area.
  • Hold cases for a minimum of 46 weeks following a transition to ensure the service user
moves between the two services successfully.


About you:


  • You will focus on engaging with people who have a history of rough sleeping and substance misuse.
  • You will have some experience of engaging with individuals with multiple and/or complex needs and demonstrate a determined approach to finding solutions.
  • Some knowledge of one or more of the following is required:
Homelessness

Mental health

Criminal Justice System

Substance misuse.
