Job overview:

Salford is currently undergoing service transformation to redesign our Home First discharge processes and pathways. Our vision for Home First is 'People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary'


We have established a Home First Multidisciplinary Community Hub to support people admitted to hospital to be discharged 'Home First' and we are looking for a nurse to join the team as a Home First Care Home Liaison Officer.

You will work with therapy, nursing, pharmacy, social work and voluntary sector colleagues to plan discharge assessments and post discharge support.

Your role will be to lead on promoting and strengthening the interface between Hospital and Care Homes, working closely with Care Homes and the Care Home General Practice.

You will ensure effective Trusted Assessment for people moving to a care home from hospital in Salford.


The Service operates over 7 days, currently covering the hours of 8am to 6pm

Main duties of the job:


You will be a registered nurse and have experience of working in hospital setting and knowledge of hospital discharge processes, plus national discharge policy.

Experience of integrated working in a multidisciplinary team and partnership work is required. You will also have good knowledge of the Care Home sector and be able to influence and motivate others.

As this is an area of transformation, innovation will be encouraged and you should have a proactive approach to identify service development opportunities, plus have resilience in the ability to adapt to a changing landscape.

Essential criteria:


  • Substantial Post Registration with experience in an Acute setting
  • In depth knowledge of hospital transfer and discharge processes
  • Awareness of the challenges of working across professional and organisational boundaries.
  • Knowledge of the care home sector
  • Experience of using IT Clinical and Administration Systems.
  • Good interpersonal skills with the ability to communicate with a range of people.
  • Ability to maintain and develop effective working partnerships within a team.
  • Ability to motivate and influence colleagues within and outside of the Team.
  • Commitment to integrated working across health and social care.
  • Ability to plan and prioritise workload.
  • Ability to provide leadership to junior members of the team.
  • Experience of complex discharge coordination.
  • Community health or care experience.

Desirable criteria:


  • Degree Level Study
  • Mentorship Qualification
  • Evidence of Continuing Professional Development
  • Knowledge of DOH guidance and legislation in relation to discharge and transfer of care.
  • Experience of working with care homes.
