We have an exciting opportunity for an enthusiastic rehabilitation assistant to work in our friendly community neurological team.

You will be working closely with your therapy colleagues to deliver high quality care to patients with stroke and other neurological conditions.

If rehabilitation is your passion then this is the job for you.

You will be part of a well-structured MDT with support from experienced clinicians within the team across all professions.

You will support patients within their home, to provide rehabilitation and guidance, with a view to promoting independence.


You will be joining a friendly, dynamic and cohesive integrated community therapy team which prides itself on close working relationships with the MDT.

Person-centred care and an approach which promotes self-management and independence are at the centre of our philosophy.

As the rehabilitation assistant, you will be responsible for implementing treatment plans created by the qualified member of staff.

You will act as a source to provide health education and advice to patients, carers and their families.

You will need to be a versatile, adaptable team player who has good organisational skills. There is a strong emphasis on CPD, reflective practice and clinical supervision within the service. Regular in service/external training is encouraged.

To assist qualified allied health professionals including occupational therapists, physiotherapists, speech & language therapists and clinical psychologists in the delivery of neurological rehabilitation in a variety of settings.


To work without direct supervision, but under the direction of the qualified staff, in the performance of specific tasks as part of their treatment and rehabilitation programmes.

This may be out in a community setting such as the patient's own home or in a sports and leisure or day care setting as a lone worker or with a qualified professional.

To implement and progress treatment plans delegated by qualified staff or following a protocol.

To assist in delegated data collection.

To pre-screen patients including gathering of information, and performing delegated outcome measure testing.

To take responsibility for a delegated area of support work including monitoring and feeding back to qualified staff.

This includes but is not limited to:

  • Stock ordering
  • Equipment safety checks
  • Information for patients and carers being kept up to date
  • To be aware of the health and safety aspects of working.
  • To implement any policies which may be required to improve the safety of the work area. This will include the prompt recording and reporting of accidents to senior staff and ensuring that equipment used is safe.
  • To comply with the organisational and department policies and procedures. To be involved in the reviewing and updating as appropriate
