This role has a starting salary of £36,282 per annum based on a 36 hour working week.

We are looking for enthusiastic and experienced qualified and registered Occupational Therapists for our new Discharge to Assess (D2A) roles within our Adult Social Care Locality Occupational Therapy teams in Surrey County Council.


About the Role


Joining us, you will be part of a multi-skilled team which includes: social workers, occupational therapists and assistant social care practitioners.

You will also work with colleagues across all sectors to arrange appropriate support in the community andwill be managing a caseload of clients ensuring a high standard of Occupational Therapy provision.


Discharge to Assess is a pathway designed to rapidly discharge 95% of people from hospital once it is medically optimal and safe for them to return home.

With this model, there is limited assessment of rehabilitation within the acute hospital setting. Oncesomeone has returned home, detailed functional assessments are completed, and ongoing care and equipment needs are identified and organised.

You will play a key role in facilitating the transition from hospital to a community setting, developing partnershipworking with healthcare providers and adult social care.

The newly created Occupational Therapy roles will sit within various locality Adult Social Care teams across Surrey.

The role involves providing specialist occupational therapy assessment and treatment to inform further care planning for clients on thispathway within the 4-6-week timeframe.


About You


This exciting new post is an opportunity for practitioners who may have worked in a healthcare setting to utilise their knowledge and skills of this sector whilst also developing their professional experience of working in adult social care.


You will have the opportunity to put into practice the legislation underpinning, and further enhance your skills in, moving and handling risk assessments and intervention, equipment provision and rehabilitation with the aim of supporting and empowering ourclients to live meaningful lives within the community.

You may also be required to undertake safeguarding work and mental capacity assessments for those under your care.


In order to be shortlisted, please evidence;

  • Ability to complete person centred and proportionate OT assessments
  • Familiarity with moving and handling equipment such as hoists and stand aids
  • Knowledge of medical conditions and the likely impact on the individual, carer and family
  • Continuing professional development. Applicants must have and maintain current registration with the Health and Care Professions Council (HCPC) and adhere to the HCPC standards for conduct, performance and ethics.
  • A full UK driving licence and access to a vehicle.
