This brand new Dietetic Assistant role will work under the leadership of the Dietitian to deliver the All Wales Diabetes Prevention Programme to a cluster region (namely Anglesey).

Working together with the Dietitian, GPs and practice nurse team in the delivery of the care to the practice population, this role holds the potential to support transforming health outcomes with the prevention of Diabetes.

This post is fixed term to 31/3/24 due to meeting the needs of the service.


This Dietetic Assistant will work under the leadership of the Dietitian to deliver the All Wales diabetes prevention programme to a cluster region (Anglesey).

Support will be given to GP practices in the appropriate identification of individuals at risk of developing diabetes and their invitation for lifestyle review.

The post holder will receive training to undergo brief intervention with eligible patients, holding diet and lifestyle conversations and signposting to local services to support a reduction the risk of development of diabetes.

The role will include the collection of a minimum data set and support of the development and distribution of written resources.

To assist the Dietitian, GPs and practice nurse team in the service and delivery of the care management of the practice population, with specific relation to the prevention of Diabetes.

The post holder will work under indirect supervision of the Lead Dietitian and undertakes task and duties delegated by the Dietitian or a suitably qualified regulated professional.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

This post is fixed term for 15 months due to - meet the needs of the service.
