Description
Manage the Service Users condition and oversee the patient's treatment plan

  • Monitor and coordinate treatment and care plans with the Service User to educate them about their illness
  • To implement Care plans for Service Users, and to update Care Plans and Risk Assessments in line with internal policies.
  • Visiting the service user to check on the care they have recieved and make reviews of the support plans and examine written records of medication administration and daily log notes kept by staff
  • Consult and collaborate with other health care providers and specialists to set up patient appointments and treatment plans.
  • To ensure that Care Quality Comission standards are met for Service Users by the team
  • Drawing up rotas to ensure the needs of all care plans are met and to ensure that the support staff receive their rotas at a reasonable and agreed time
  • To report issues regarding and concerns with the services provided by the staff such as safeguarding and confidentiality breaches
  • To always act in a manner which represents the confidentiality Service Users, Staff and any other involved parties.
  • To represent the company in a professional manner at all times
  • To carry out assessment of new/potential clients
  • Ensuring Health and Safety compliance
  • Ensuring that all activities such as personal care and medication administration meet the required standards
  • Ensuring that all care calls and shifts are covered and will be required to fill in when required
  • Ensuring that the staff deliver highquality and personalised care to patients
  • Planning and utilising resources and staying updated with the clinical knowledge to esnure that patients receive the best care
  • Recruiting, training and supervising staff and communicating the needs of the patients to Medical Staff

Job Types:
Full-time, Part-time, Permanent, Fixed term contract

Contract length: 12 months

Part-time hours: 21 per week


Salary:
£18,000.00-£22,000.00 per year
