Following a change to the way we structure our Adult Social Care teams in Cambridgeshire, the Adults Community Teams have an exciting, new, permanent opportunity for you to join us as an Adult Support Coordinator.

You will initially be working with our Post Hospital Discharge Review project team and will move into one of our locality Community teams when the project concludes.


We're embracing new ways of working so our social work teams can work where they need to and have the chance to be part of a collaborative team environment with strong working relationships with health professionals and partners to achieve better outcomes for the adults we support.

Interviews will be held by Microsoft Teams, on 6th July 2023.

What will you be doing?

You'll be liaising with other departments, undertaking Social Care Assessments and Reviews, Mental Capacity Act Assessments, as well as care and support planning to achieve better outcomes for the adults we support.


The project team has a focus on increasing independence and wellbeing once an individual has been discharged from hospital through the use of social networks, the voluntary sector and the wider Health and Social Care multidisciplinary teams.

About you

You will have some general experience and understanding of the health and social care sector with a genuine desire to support people: you'll put the individual at the heart of everything.

You'll need to be inquisitive and provide evidence based, proportionate recording with a keen focus on strength-based practice.
