Are you a compassionate person interested in working with older people?
Do you want to use your knowledge and experience of health and social care services in a new way?

With flexible community-based working, you will get to visit people in their own homes and work with them in a holistic way to support them at a difficult time.

You will do this through spending 46 weeks with each client:
developing goals around what matters to them

- liaising with professionals and family members to ensure that services and support are in place and working effectively
- organising short-term practical support in the home
- providing emotional support through regular visits or calls
- linking the client to other services which can support their health, independence and wellbeing in the longer term
