Job advert- To provide a direct service to carers and families of people with learning disabilities and/or those with an autism spectrum condition (ASC) under the Wokingham Carers Partnership Service and to work alongside the Promise Inclusion (Pinc) Family Liaison TeamThis service will include:
- Identifying and recognising carers including seldom heard carers
- Empowering carers
- Enabling carers to have a life outside their caring role
- Supporting Young Carers
- Main duties include:
- To provide information and advice to unpaid carers in Wokingham and where necessary to signpost and refer onto other relevant services including the Pinc Family Liaison Workers.
- To complete selfassessments and identify the needs of carers and work with them to meet these needs, including supporting them to make emergency plans.
To log and keep up-to-date and accurate records on our CRM system (currently Charity Log)- To maintain a library of information and knowledge to be able to give up-to-date information on topical and relevant issues.
- As part of the Wokingham Carers Partnership to work with partners to initiate, plan, organise and host events for carers including events during Carers Week and for Carers Rights Day.
- As part of the Wokingham Carers Partnership to work with partners and liaising with the Pinc team to initiate, plan, organise and host respite events for carers.
- As part of the Wokingham Carers Partnership to work with partners and liaising with the Pinc team to initiate, plan, organise and run workshops for carers.
- As part of the Wokingham Carers Partnership to organise and host support groups for carers for peer support and awareness of Carers Rights and to organise a Carers Forum.
- To monitor the outcomes for carers of the work carried out with them and collect feedback.
- To liaise and work with the Promise Inclusion Family Liaison Team and refer onto them as necessary.
- To liaise with Children's and Adult Social Services including the Transition Officers at Wokingham Borough Council.
- To provide reports on a regular basis to the Board of Trustees, Wokingham Borough Council and other agencies as required.
- To attend meetings as required and deputising for the CEO including the Carers Strategy Meetings and other Carers Forums.
- To develop and maintain good partnership working with key organisations and partners (Royal Mencap Society, statutory, voluntary and community groups) and the general public.
- To research/keep uptodate with local and national issues/matters of relevance to clients (e.g. legislative, service provision changes, support available etc), communicating this as necessary.
