We are looking to appoint a self-motivated individual with initiative, drive, strong interpersonal, admin and IT skills to work as a Palliative Care Coordinator based at The Priscilla Bacon Lodge Hospice, Norwich

Coordination Duties:

To work as part of the team for the Specialist Palliative Care Service liaising closely with clinicians to ensure a timely response to referrals.

Taking and recording referrals electronically and allocating care packages under the supervision of the lead clinician and working alongside the integrated teams and a variety of organisations and care providers.

Call Handling Duties:
To deliver an effective, safe and competent call handling service for callers to the Specialist Palliative Care Service which includes patients, their representatives and Healthcare professionals
