We are looking for an experienced Incident Response Officer to join our team at Cygnet Health Care.

Cygnet Health Care have been providing a national network of specialist mental health services for more than 30 years.

This role be working on a full time, 37.5 hour per week basis.

The role is remotely based however travel to our London office or services across the nation will be required on occasion.

The Role:


To be responsible for promoting a Cygnet wide learning culture using areas of improvement identified through the outcomes of safety incident responses, under the requirements of the Patient Safety Incident Response Framework (PSIRF).

The post holder, working directly with the Group Risk & Incident Manager, will be responsible for ensuring that all responses, practices and systems contribute to reducing harm and improving safety whilst ensuring effective monitoring of outcomes and learning.


Your Day-to-Day

  • Supporting the ongoing implementation and development of the Patient Safety Incident Response Framework plan
  • Build and maintain a portfolio of safety incident responses and their outcomes
  • Manage the incident response portfolio ensuring that all incident responses provide an outcome which informs learning and improvements
  • Support the weekly safety response and complex case group
  • Supporting local units and regional/ corporate governance with outcome reports and valuable sharing of learning and safety improvements.
  • Supporting the Group Risk and Incident Manager with the monitoring of incident data and concerns, identifying trends to inform a safety incident response.
  • Work with the DATIX administrator to build notifications to fit the requirements of safety and incident responses.
We are looking for someone who is...

  • Educated to a degree level or equivalent
  • Excellent understanding and working knowledge of the Patient Safety Incident Response Framework
  • Ability to structure and write a comprehensive report for the purpose or satisfying the relevant group or committee.
  • Good level of interpersonal and communication skills, including the ability to communicate to different audiences of knowledge and seniority.
