We are looking for an experienced and enthusiastic transformation practitioner to join our team, and help us push forward our strategic projects across two acute hospital sites in Macclesfield and Stockport.

This post is a fantastic opportunity to work at the forefront of designing and implementing collaborative, multi organisational change.

The Service Transformation Practitioner will work closely with the Programme Manager and wider team to plan, lead and delivery proposed service change, improvement and benefits realisation of health and care services.

Leading to improved health outcomes and wellbeing of the patients we serve.

37.5 Hours per week

12 Month Fixed Contract

Lead the development and delivery of effective and efficient programmes of work in order to drive continuous quality improvement and transformational change.

Develop strong collaborative working relationships and wide networks with internal and external stakeholders to support the delivery of transformation/improvement and/or redesign programmes.

All staff will be expected to work across departmental boundaries in a collaborative, matrix style approach, providing a 'consultancy' function to the Service Lines
