Description

The post holder will provide clerical and administrative support to enable the Continuing Care team (Continuing Healthcare / Complex Care) to deliver an efficient and effective service, in accordance with the National Framework for NHS Continuing Healthcare and NHS-Funded Nursing Care October 2018 (Revised) and other relevant local and national guidance.

This will involve working closely with all members of the Continuing Care team as well as liaising on a daily basis with members of the public, including patients and their representatives and other professionals (including GPs, Local Authority colleagues and providers.
