Job summary

This role involves working with individuals who have complex health and social care needs. The post holder will be responsible for providing short-term support to vulnerable individuals upon leaving custody or court. They will work closely with the individual to prioritise their needs and plan actions to address those needs.

The role involves ongoing assessment, accessing, and implementing a wide range of care services across an ageless service. The post holder will also be responsible for carrying out assigned tasks as part of the Health and Justice Service.

The job requires experience working with individuals with complex health and social care needs, knowledge of the Criminal Justice System, and ability to form rapport with individuals and groups.
