Overview
We are very excited to be recruiting for a Paramedic to join our forward thinking, experienced team.


This role will be employed by The Chalfonts Primary Care Network (CPCN) which includes three GP Practices and supports a local population of approx.

31,000.


This role will ideally be suited to an experienced Band 7 Paramedic, with a proven background in delivering advanced level patient centred care within a primary care setting.

Though we will consider applicants who are considering a move into primary care, who are enthusiastic and passionate about providing high quality care to our patients and becoming an integral part of our experienced multi-disciplinary team (MDT) who can support a safe & enjoyable transition.


About Us
The Chalfonts Primary Care Network (PCN) is made up of the three GP practices based in Chalfont St


Peter, Buckinghamshire These are:
The Allan Practice, The Hall Practice and The Misbourne Surgery.

Since the NHS was created in 1948, the population has grown and people are living longer.

Many people are living with long-term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.


To meet these needs, practices have begun working together and with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in primary care networks.


Primary care networks build on the core of current primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care.

Clinicians describe this as a change from reactively providing appointments to proactively providing care for the people and communities they serve.

Where emerging primary care networks are in place in parts of the country, there are clear benefits for patients and clinicians.


About the Job

The role will involve working as an autonomous, accountable Paramedic, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment to deliver quality patient services.

The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills.

There will be the need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.


The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within General Practice.


Key Duties and Responsibilities

  • Alongside the medical team, to provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problemsolving and clinical decisionmaking to establish a diagnosis and management plan.
  • To provide assessment, treatment and diagnosis at point of first contact by attending to patients in a variety of clinical or nonclinical settings according to patients' needs.
  • To undertake home visits in accordance with the relevant protocols
  • To make professionally autonomous decisions for which he/she is accountable.
  • To refer patients to an alternative care setting or treat and discharge as appropriate.
  • Advise patients on general healthcare and promote selfmanagement where appropriate, including signposting patients to other community or voluntary services, PCN services and to selfcare.
  • Perform specialist health checks and reviews.
  • Perform investigatory procedures as required.
  • Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.
  • Perform investigatory procedures needed by patients and those requested by GPs and the wider PCN.
  • Perform a domiciliary (home) visit.
  • Support the delivery of anticipatory care and end of life care plans and lead certain community services (e.g., monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing).
  • Provide an alternative model to urgent and same day home visits for the PCN and undertake clinical audits.
  • Communicate at all levels across PCNs and other organisations, ensuring effective, patientcentred service.
  • Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings as required and accepting referrals and referring to specialist services where appropriate.
