An opportunity has arisen for an enthusiastic and motivated individual to join our team to support the community therapy team within the Central Shropshire Locality.


The service consists of multi-disciplinary team including Physiotherapists, Occupational Therapists and Rehabilitation Assistants, also working closely with our nursing colleagues and social care practitioners to provide a patient centred approach.

The aim of the team is to promote independence through rehabilitation and crisis management with patients in their own home or within a short term care setting to prevent unnecessary hospital admissions and support timely hospital discharges.


We can then provide on-going rehabilitation within the patient place of residence to improve, maintain or help to manage underlying conditions.

Our aim is to optimise everyone's potential to gain greater independence to add quality to their life.


You will be involved in providing and evaluating goal focused rehabilitation and enablement programs, as well as complex discharge planning alongside our therapy and nursing colleges both in the community and community bed base environment.


The post holder will also undertake triage of patients being referred to service and hold an independant caseload within the competency framework appropriate for a band 4.

The post holder must have a current driving license and access to a car for work.

We are looking for a conscientious individual to work with the established therapy teams. We are a dynamic, developing organisation with well recognised rehabilitation services providing education, advice, support and rehabilitation for patients.

You will need to demonstrate high standards interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

  • Work under direct and indirect supervision of a Registered Health Professional to provide health care support to an identified caseload
- this may be in a community setting (pathway 2 beds) or the patient's own place of residence.

  • To work within agreed protocols within the designated work area.
  • Maintain agreed, acceptable standards of care and only undertake duties within own personal competence following training/education and assessment of competency.
  • To provide physical, emotional, psychological, social, cultural and spiritual care to patients under your responsibility
  • Following delegation from the Registered Healthcare professional, in collaboration with the patient and or family/carer as appropriate take responsibility for undertaking
- necessary patient assessments
- development of a plan of care
- evaluate and review the effectiveness and outcomes of planned care interventions
- communicate any essential information to the Registered Healthcare worker as necessary
