Are you passionate about promoting the well-being of older individuals and helping them lead empowered, independent lives? Age UK Kensington & Chelsea (AUKC) is seeking a dedicated Health and Social Prescribing Co-ordinator (HSPC) to join our innovative My Care My Way (MCMW) team.

MCMW is transforming care for individuals aged 65+ with complex needs, providing holistic support rooted in evidence-based practices.

Role Summary


As an HSPC, you'll play a vital role in coordinating care for older patients, focusing on promoting independence and self-care.

This position blends responsibilities typically held by Health Care Assistants, Care Navigators, Social Care Trusted Assessors, and more.

Your duties will range from basic clinical tasks, like blood pressure checks, to supporting patients' access to community services that align with their specific needs and goals.

Essential Criteria

  • Ability to build strong stakeholder relationships with GPs, case managers, carers, and patients.
  • Flexibility to work remotely and independently, including from various locations such as GP practices, hub offices and in the community.
  • Strong problemsolving skills to match complex patient needs with available local support and services.
  • Proficiency in IT tools like SystmOne (NHS IT system), Excel, Outlook, and online research.
  • Health and social care experience.
  • Degreelevel education or equivalent qualifications.
