The Stroke and Neuro Service are looking to recruit an individual to join our team on a Fixed Term 21 Month Apprenticeship contract working 37.5 hours per week while undertaking a Level 5 Assistant Practitioner Apprenticeship aiming to achieve an Assistant Practitioner qualification.
Initially you will commence employment at Band 3, upon successful completion and evidence of your qualification and competencies you will then progress to a Band 4 Rehab Assistant Practitioner.
The primary function of this role is to work with people who have stroke and neurological conditions, providing a high standard of rehabilitation supporting physiotherapy, occupational therapy and speech therapy programmes in the community and on the stroke unit, including provision of some initial assessment.

- Training towards assistant practitioner qualification e.g. Level 5 Assistant Practitioner Apprenticeship
- The community team provides an early support discharge service for people with stroke and ongoing therapy for people with stoke and neurological conditions
- There is an expectation that the post holder will see an average of at least 5 community visits per day with a higher number of contacts when seeing people as outpatients
- This role includes shifts rostered to work Saturdays, Sunday and Bank Holidays
The Stroke and Neuro Services consist of:
- George Earle ward, Acute Stroke Unit in Torbay hospital with 28 beds providing acute assessment and intervention.
- Templer ward, Inpatient Rehab Unit at Newton Abbot Hospital which has 15 stroke beds and 5 Neuro beds and provides postacute inpatient neurological rehabilitation prior to discharge to the community.
- The community team provides ongoing care in peoples own homes and in the outpatient department. The service is part of the continuum of care in South Devon and has been recognised nationally as an example of good practice

You must have a current driving licence and be able to transport yourself and occasionally equipment by vehicle within the community
