    Main duties of the job


    The Home Treatment Team (HTT) provides a 7 day a week service, between the hours of 8am and 10pm, alongside a twilight shift which is 5.30pm until 3am for gatekeeping.

    Our key responsibilities are around the assessing and delivery of care for people who are in crisis with their mental health. The team is multi-disciplinary and has a strong ethos of team working and team support, in order to deliver high quality care. The post holder will work on a shift basis over a 7 day period. The HTT works closely within the crisis network in ensuring that good liaison and joint working with other crisis provision teams are maximised.

    The band 6 nurse / social worker will work within the Home Treatment Team. The post is to provide clinical care to Service Users who are in Crisis. The HTT is multidisciplinary and will involve working alongside other clinical professionals as well as the Crisis Network and other Community Mental Health Teams. As a band 6 the role will involve supervision of junior staff and participation in team development.

    You will be involved in the assessment of Service Users in Crisis, leading on crisis assessments and decision making with onward referral to appropriate teams.

    Planning interventions, delivering a variety of Acute Crisis interventions, whilst reviewing care and planning for discharge.

    You have the opportunity to work as a shift coordinator, the newly formed gatekeeper role and as an interface between ourselves and the wards.

    Job responsibilities


    Shift patterns are:

    • Twilight to 03.00 hrs with 30 minutes break
    • Days hrs with 30 min break
    • Late to 22.00 hrs with 30 minutes break

    Clinical

    • Triage and determine urgency of new referrals to the service
    • Carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree the most appropriate course of action
    • To assess, evaluate and manage clinical risk
    • To formulate a plan of care based on identified need
    • Deliver evidence based crisis interventions as appropriate
    • Review effectiveness of care plan at regular intervals
    • To work collaboratively with service users, carers and partner agencies
    • To coordinate the clinic work of a day by day basis
    • To maintain computer held care records in an accurate and timely fashion
    • Undertake all work in a culturally sensitive manner
    • Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.
    • Develop effective working relationships with inpatient, community and voluntary organisations.

    Supervision

    • Regularly review own clinical work with supervisor to promote efficacy and contribute to the overall management of the HTT
    • Deliver supervision and facilitate annual PDR with more junior members of staff
    • Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution

    Education and Training

    • Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain sign off status
    • Participate in training relevant to role, including training needs for the team.
    • Promote insight and understanding of mental health issues to service users, carers, and other agencies
    • Contribute to audit, research and evaluation as appropriate

    Service Development

    • Contribute to service development and review of development within the team.

    This is not an exhaustive list of duties and responsibilities and in discussion with the manager the postholder may be required to undertake other duties which fall within the Banding of the post. The job description will be reviewed regularly in the light of changing service requirements and any such changes will be discussed with the postholder.

    The post holder should be comfortable in the use of computer technology, have basic keyboard skills and have the ability to use email.

    Safeguarding Children and Adults

    Qualifications

    • Registered Nurse - Mental Health
    • Experience of working as a Band 5 or 6 Mental Health Nurse in a community and MDT setting
    • Evidence of continued professional development
    • Registered Social Worker in Mental Health
    • Experience of working as a Band 5 or 6 Mental Health Social Worker in a community and MDT setting

    Knowledge & Skills

    • Working knowledge and understanding of Care Programme Approach & Care Management
    • Knowledge and understanding of appropriate therapeutic approaches
    • Knowledge and understanding of PDR and supervision processes as a supervisee
    • Demonstrable risk assessment and risk management skills
    • Demonstrable caseload and time management skills
    • Ability to work autonomously as well as a co‑operative and effective member of the multi‑disciplinary team with an ability to communicate at all levels
    • Ability to assess (under supervision) plan and evaluate care
    • To have worked in crisis and acute care
    • Ability to work autonomously, as well as part of a team
    • Understanding of Local and national policies
    • Ability to network and to manage the interface between organisations involved, utilise and contribute to professional networks

    Experience

    • Experience of multi‑disciplinary team working
