Are you passionate about mental health? Croydon rehab and recovery services is looking to recruit a Recovery Team Leader to join our specialist Mental Health Recovery Pathway Service.


We are seeking an enthusiastic and communicative person with experience working with vulnerable people who experience complex mental health problems in one-to-one and group settings, and excellent skills in service user assessment and crisis management (including dealing with conflicts and strong emotions), alongside a broad range of approaches to advocacy.


The role is all about improving outcomes for this patient group, working creatively with patients, carers and placements in the borough to co-create care and risk plans that will help them achieve their goals and to live as independently as possible and to move on from supported, funded accommodation, to live as independently as they can.

This key to success is often about building effective partnerships with GPs, Crisis services, housing and other third sector providers

  • Management of the team and support rehabilitation working in partnership with care providers.
  • Work closely with the Review and SLP Review Teams which focus on the review and stepdown of service users and ensuring that individuals are best placed to meet their needs.
  • Support the Service Manager to ensure the delivery of a pathway for people using services that meet all quality, financial and operational performance targets.
  • Be responsible for ensuring the provision of safe services for all clients under the services.
  • Develop, coordinate and implement a process for managerial supervision, clinical supervision, case load management and staff annual appraisal.
  • Be responsible for recruitment and retention and work with team colleagues and external agencies to ensure that people who use the service and their families or carers are involved and development and delivery of services.
  • Working in partnership with service users, carers, the wider multi-disciplinary team and other partners and agencies to ensure effective outcomes
  • There is also the opportunity to get involved with some of the strategic planning of placements within the borough in partnership with social care, and CCG/SLP.. The Team has a role in supporting the Local Authority and CCG to achieve financial efficiencies.


The Recovery and Rehabilitation Team is a community based integrated Health and Social Care team which sits in SLAM, focused on supporting service users in Croydon funded residential or 24 hour supported living placements in the community.

The team aims to ensure service users are supported to develop skills for more independent living and to then support them to Step-Down to more independent settings using rehabilitation care planning in close partnership with the care providers.


Currently the team work with service users with Complex Mental Health needs with a primary diagnosis of Psychosis but this may change as community transformation projects within Croydon change the way services are delivered to meet the needs of the local population.

The post holder carries day to day responsibility for the operational management and delivery of services within the pathway:

  • Working in partnership with service users, carers, the wider multi-disciplinary team and other partners and agencies to ensure effective outcomes
  • To provide leadership and support the Service Manager in ensuring high quality care is provided across the pathway
  • The post holder, with the support of the Service Manager, will hold delegated responsibility for the management of the pathway team budget.
  • The post holder will have responsibility for ensuring performance within designated trust standards including Key Performance Indicators and financial management targets.
