Description

We are looking for an enthusiastic, motivated and adaptable Band 7 Physiotherapist to join our front door and frailty (FDFT)team on a 1 year contract to cover maternity leave and provide excellent patient care in an environment where staff are supported to innovate and lead improvement.


You will be based at Worthing Hospital within the Emergency Floor and frailty wards focusing on acuity rapid assessments and facilitating discharges, providing excellent patient flow within our Trust.

You will be well supported by the highly experienced team-leads with excellent opportunities to develop your own clinical and leadership skills, working within a strong peer group of other physiotherapists and occupational therapists alongside a strong and supportive multi-disciplinary team.


Worthing Hospital is an attractive seaside location on the South Coast, situated adjacent to the South Downs National Park, 20 minutes from the City of Brighton and Hove, with excellent transport links to London and across Southern England.

You will be joining a strong and supportive Physiotherapy Department within the top performing University Hospitals Sussex NHS Foundation Trust.

To be responsible for the training and development of the staff working in the FDFT (Emergency Floor and Department of Medicine for the Elderly wards) therapy service including weekend provision.


To comprehend the acuity of patients within the front door and ward setting, understanding, and facilitating complex hospital discharges ensuring patient flow.


To lead in the front-door advanced assessment, clinical diagnosis and treatment of patients who may have complex and/or chronic presentation, under different specialities.


To lead in the Department of Medicine for the Elderly (DoME) ward advanced assessment, clinical diagnosis and treatment of patients who may have complex and/or chronic presentation.


To provide an effective high-quality service to all patients and assist with the supervision and teaching of all members of the team including students.

To provide skilled team leadership and service development within therapies and the wider MDT.


To implement policy and policy changes in own clinical area and to assist with the setting and monitoring of standards of practice.


To undertake evidence-based projects in conjunction with the Head of Physiotherapy and other members of the MDT, including recommendations for change in practice and to lead in appropriate departmental research in this clinical area.

To problem solve clinical and non-clinical complex situations.

To link day to day working with other physiotherapy specialties within the FDFT service.

Communication


To communicate effectively and work collaboratively with medical, nursing and other professional colleagues to ensure the delivery of a co-ordinated, patient centred multidisciplinary service.

This includes highly complex information in a lead role at discharge planning (e.g. family meetings, goal setting, team case conferences, and complaint meetings).

To work collaboratively with Occupational Therapy A&E Team Lead in order to deliver and effective and efficient front door therapy service.

To communicate effectively with external organisations such as community services, GP's etc to help ensure effective transfer of patient care.

To contribute to discussions with external agencies on systems in place to facilitate this.
