Description

The Grange is a 17 bed step down and reablement facility, Supporting patients discharged from community and general hospitals. Our patients are of adult ages 18+ and can have various physical, emotional and psychological support needs. We offer short term reablement to assist patients back onto their feet and hopefully return home.

Senior Support Worker required for well established and respected step down facility.

Shift pattern comprises of 12 hour shifts on a two week rolling rota including alternate weekends.

Job Specification:

To support patients upon discharge from hospital within the step down facility.
Plan and introduce an effective and robust rehabilitation plan for patients to return home as safely and speedily as possible.
Encourage and promote independence with patients living with prolonged health concerns.
Partake in the admission and assessment of patients from community and general hospitals within the region.
Partake in the safe discharge of Patients home or to other support facilities.
Support and guide a team of support workers.
Create and maintain support plans and patient profiles using an electronic support and care system.
Administer to and audit medication for patients within the service.
Reable patients to secure community bonds where applicable.
