An exciting opportunity has arisen for a motivated and skilled Mental Health Nurse, Occupational Therapist or Social Worker to join our top performing Early Intervention in Psychosis Team in Bolton as a Care Coordinator.

You will be joining a client centred, dynamic and friendly team, who prides themselves on delivering high quality, evidence based interventions for those experiencing first episode psychosis.


The Early Intervention in Psychosis Team (EIT) works with people years old) who are experiencing a first episode of psychosis.

It is a multidisciplinary team consisting of staff from a variety of disciplines, including Psychiatry, nursing, social work, occupational therapy, and psychology.

One of the key aims of the team is to reduce the duration of untreated psychosis.

We achieve this through offering full care-coordination, evidence based group and individual psychological interventions (including cognitive behavioural therapy and Family Interventions), support with medication management, support with accessing employment, education and vocational activities and support for families and carers.


The role of the Care Coordinator is integral to supporting a person on their recovery journey, through the CPA process.

  • To hold and manage a caseload of service users who have
been identified as experiencing their first episode of
psychosis, including needs assessments, care planning and
delivering psychosocial interventions.

  • To complete specialist assessments for referred service
users accepted into the service when required.

  • To undertake risk assessment and risk management for
individual service users and to provide advice to other
professionals on acute and chronic aspects of risk
assessment and risk management.

  • To work as part of our integrated MDT, and to participate in
a clinical decision making process to determine the most
appropriate management plans for all service users who are
under the care of the team.

  • To provide advice, guidance and consultation to peer and
junior professionals within the service, both individually, and
by attendance at team referral and review meetings.

  • To participate in the duty/assessment rota operated by the Early
Intervention Team.

  • To complete specialist assessments for referred service
users accepted into the service when required.

  • To maintain a high standard of clinical record keeping, including electronic data entry and recording, report writing and the responsible exercise of professional selfgovernance in accordance with relevant professional code of practice and Trust policies and procedures.
  • To have knowledge of, and contribute to the team adherence to the NHS England Access and Waiting Time Standards for EIP.
  • To undertake risk assessment and risk management for
individual service users and to provide advice to other professionals on acute and chronic aspects of risk assessment and risk management.

  • To liaise professionally and sensitively with other involved parties in order to provide seamless and consistent high quality care for service users under care of the team. This includes, but is not limited to; carers, families, internal and external colleagues, and voluntary services as appropriate.
  • To hold and manage a caseload of service users who have
been identified as experiencing their first episode of psychosis, including needs assessments, care planning and delivering psychosocial interventions.

  • To have a good knowledge of the required documentation for case management, and to ensure compliance with any associated performance standards.
