We are looking for a Referral Co-ordinator to join our busy team. The post holder will register and co-ordinate all referrals to the Cardiac Surgery Team. The post holder will work within a range of policies and procedures escalating issues with capacity as appropriate.This post holder will also attend multidisciplinary team meetings, recording the outcomes from the meeting and ensuring that these are escalated as appropriate.
There will also be an element of cross-cover with the waiting list team.

Ensure the the referrals are allocated appropriately, as per the medical decision.
Ensure that all appropriate tests and documentation is complete at the point of receiving a referral.
Liaise with the secretarial teams re appointments and clinic capacity.
Escalation of any delays to the patients pathway.
Support the identification and escalation of any issues which compromises delivery of the 18week RTT pathway, e.g. lack of capacity
Ensure Trust systems are updated with patient pathway status information and that data quality is maintained
Carry out any duties consistent with the post/banding
To escalate shortfalls in capacity to the Directorate Management team in line with policies and procedures
