About the Role:


The Colte Partnership have an exciting new opportunity for an enthusiastic team member to become a Care Co-ordinator at Colne Medical Centre.


The Care Coordinator's role will support the primary care professionals in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred.

The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patient's journey through primary care.

The role is full time and the hours will be between hrs per week


As a care coordinator, you will be involved in proactive, person-centred care planning with people, overseeing and coordinating their care when needed, by bringing together the different specialists whose help the individual may need.


You will also help ensure that the Practice is meeting all required standards (including QOF) for the management of health conditions.


You need to have a good telephone manner when talking to patients, and a good understanding of our appointments and EMIS systems.

In June 2017 The Colte Partnership was formed consisting of 9 local practices across Colchester and Tendring caring for approx.

95,000 patients.

We also work as three collaborative Primary Care Networks (PCN's) which span across Colchester and Tendring:

  • Ambrose Avenue Group Practice, Riverside Medical Centre & Ardleigh Surgery
  • Mersea Island Medical Practice, Tiptree Medical Centre & Rowhedge Medical Centre
  • Walton Medical Centre, Colne Medical Centre & Wivenhoe Medical Centre


There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix.

You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with on-going professional development (training support, CPD and peer support). The role will contribute to the improving quality of care of our patients across the network of practices.
