Liverpool First PCN is looking to employ up to an additional two social prescribing link workers to work across several of its 11 member practices under the

Additional Roles Reimbursement Scheme:

Abingdon Medical Centre, Benim Medical Centre, Bigham Medical Centre, Dr Jude's Practice Picton, GP Practice Riverside, Hornspit Medical Centre, Rocky Lane Medical Centre, Sandringham Medical Centre, Stopgate Lane Medical Centre, Walton Village Medical Centre and Yew Tree Medical Centre.


Social prescribing link workers make a difference to patients' lives by giving patients time to focus on what matters to them and by signposting patients to community groups and statutory services to enable patients to achieve what matters to them.


Duties of the post-holder:


  • Taking referrals from the PCN's Core Network Practices and from a wide range of agencies to support the health and wellbeing of patients.
  • Assessing the extent to which patients' health and wellbeing needs can be met by services and opportunities available in the community.
  • Coproducing simple personalised care and support plans to enable patients to address their health and wellbeing needs.
  • Evaluating the extent to which actions in the care and support plan are meeting the patient's health and wellbeing needs.
  • Providing personalised support to patients and their families to take control of their health and wellbeing, live independently and improve their health outcomes.
  • Developing trusting relationships by giving people time to focus on 'what matters to them'.
  • Taking a holistic approach to support patients assume employment, training and welfare support.
  • Exploring and support access to personal health budget where appropriate.
  • Managing and prioritising their caseload insofar that the health and wellbeing needs of their population are met.
  • Referring patients back to other health professionals within the PCN where appropriate.
  • Attending monthly supervision meetings with the postholder's line manager.
  • Ensuring referrals are accurately coded and recorded in GP systems.
  • Raising safeguarding concerns with a relevant GP.
  • Drawing on the strength and capacity of local communities to enable local Voluntary, Community and Social Enterprise organisations (VCSE) and community groups to receive social prescribing referrals.
  • Working collaboratively with local VCSE.
  • Educating clinical and nonclinical staff on services within the PCN and how patients can access them.
  • Ensuring patients are referred to organisations, which have basic safeguarding processes in place for vulnerable individuals.
  • Working in partnership with commissioners, social prescribing schemes, local authority and voluntary sector leaders to collaboratively support patients.
