NEWCIS Offices Rhyl & Glan Clwyd Hospital but may be required to travel to cover other sites and events as required.
PURPOSE OF ROLE:
To provide a comprehensive service for carers in offering them information, advice, signposting, assistance and assessment to enable carers to obtain their full rights and be supported with their own well-being; under the framework of the Social Services and Well-being (Wales) Act 2014.
Provision of direct support to carers accessing community hospitals in the Denbighshire areas and the acute hospital in Bodelwyddan in order to assist prevention of carer breakdown and help facilitate timely discharge from hospital and prevent re-admittance.
Facilitation of assessments will be required, which adopts a strengths based holistic approach to a carers life and circumstances, will aim to empower and support the carer with their own personal well-being outcomes.
MAIN DUTIES AND RESPONSIBILITIES:
Assist Community & Acute Hospitals in Denbighshire to maintain information and support to carers and continue to raise awareness of carer issues within practices/Community Hospitals.
Provide information, advice and signposting on a range of services and community support available to the carer, linking to the resources of DEWIS, NEWCIS and Flintshire SPoA.
To inform all carers of their entitlements under the Act, including their right to an assessment of their needs and encourage them to take this up.
To promote the service and encourage other services to undertake carer needs assessments.
Adopt the 'what matters' approach to assessments, which is a strength based holistic assessment that empowers and supports carers to achieve their well-being outcomes.
Encourage Carers who are looking after someone with a long-term condition to register with NEWCIS which will enable them to receive long term support.
Inform carers of the NEWCIS training, counselling, group activities and volunteering opportunities for carers and past carers to enable them to receive peer support whilst developing their skills and confidence.
Participate in collaborative events with statutory services, voluntary and private sector to increase understanding of what maintains carers health and wellbeing.
Facilitate opportunities for carers to meet and receive support to help them understand and cope with the loss of a loved one.
Ensure regular monitoring and review of the service's activity, together with the collection of evidence / data to enable the evaluation of the service's outcomes, this will include adhering to the quality standards required by Charity Excellence, reporting and providing regular updates on progress of the work to the Operations Manager in accordance with NEWCIS and Funders reporting requirements.
Maintaining and ensuring confidentiality over personal information relating to individual users of NEWCIS and company information as per the General Data Protection Regulation 2018.
It is the general duty of all employees to assist with Health and Safety processes and monitoring in the NEWCIS Carers Centre on a regular basis and assist with key holder duties.
