An exciting opportunity has arisen for a highly motivated individual to join Norfolk Community Health and Care as a Band 4 Peripatetic Discharge Co-Ordinator, to support and work across 6 Generalist Inpatient units (Wymondham, Dereham, Norwich, Kelling, Swaffham and North Walsham).

You will provide support to the Discharge Co-ordinators currently in post across these units.

You will work with the multidisciplinary team liaising with family, patients and carers to plan and facilitate timely discharges from any unit.

You will be expected to travel to all 6 inpatient units to cover periods of annual leave/absence or support as an additional Discharge Co-ordinator on wards where it is required.

This post is a substantive post for 30 hours per week, Monday

  • Friday

Duties include:
Making onward referrals to appropriate services. Liaising with patients and family. Forward thinking about discharging a patient from when they are admitted. Working within a multi-disciplinary team including Nurses, Doctors, Therapists, Social workers. Problem solving and attending daily ward meetings.


This is an exciting opportunity for an individual who likes to be proactive and work as part of a team.

On a daily basis you will attend the red-green meetings to discuss patients and their discharge plans. You will take tasks/actions from this to complete. This may include liaising with family/other professionals, completing referral forms, speaking to patients, chasing up referrals, ordering transport.


You will need to be able to prioritise work and work unsupervised, as well as being part of the team.

You will need good communication skills and be organised.
