Virtual Fracture Clinic Co-ordinator Grade: Band 6 Professionally Accountable to: Stacy Waldron Divisional Head of Nursing for Surgery Reports operationally to: Tracey Dennehy Lead Practitioner, Orthopaedics and Trauma Clinically responsible to : On Call Orthopaedic Consultant Tracey Dennehy Lead Practitioner, Orthopaedics and Trauma Operationally accountable to: Cherelle Crooke Directorate Manager T&O Job Purpose The purpose of this role is to ensure complete co-ordination of the virtual fracture clinic sessions.

The VFC co-ordinator will assist the on call orthopaedic consultant in each patient consultation ensuring clear plan of care which is communicated to the patient.

The VFC co-ordinator will give information, advice and provide a source of knowledge for patients.

This will reduce the amount of patients requiring attendance into face to face clinics and ensure appropriate treatment pathways for patients to ensure patient centred working, improve patient care and patient satisfaction.

The VFC co-ordinator will work with the team to develop the service for the future.

Key role and responsibilities To co-ordinate the VFC session and assist the orthopaedic consultant in communication of the plan of care for each individual patient.

To ensure all patient records are available for the VFC consultations Prioritise appointments depending on the outcome of conversation with patients.

To document and coordinate accurate plans of care for patients and ensure follow up appointments are made and communicated with patients To give patient information and clinical advice, on the telephone, following consultant review in the virtual fracture clinic empowering patients to manage their own conditions and ensure VTE prophylaxis is assessed and prescribed for appropriate patients.

Arrange onward referrals to speciality orthopaedic consultant and clinic appointments and communicate these with patients.

To produce clear accurate documentation in patient records To arrange further investigation such as x-ray, MRI, Ultrasound and communicate appointments with patients To develop effective networks and channels of communication across multi-professional groups.

To ensure good channels of communication between Consultants, Emergency department staff, TNPs, medical secretaries and directorate management To assist in presentations to educational meetings, seminars and symposia as required They will drive the process of the VFC and work ensure the process is robust.

The VFC co-ordinater will ensure governance procedures are met and communicate any issues through the departmental governance process.

To work in collaboration with other therapists to audit the service we provide and set out and implement improvement strategies The post holder will work autonomously but collaborate with the multidisciplinary team

Consultant orthopaedic surgeons and their teams, A&E consultants and their teams, surgical care practitioner, Matron, nursing staff, theatre staff, physiotherapists and occupational therapists, bed management team, the surgical co-ordinator, anaesthetic staff, directorate manager, External: General Practitioners, Nursing staff in Community Hospitals

Contact with bodily fluids Infection control Visual display unit operation Covid Risk Job Summary Clinical and Audit Facilitates the process of the VFC on the Worcestershire Royal Hospital to ensure all patients seen within the clinic have robust treatment plans that is communicated with them.

Works closely with the on call T&O consultant to develop treatment plans Has health assessment skills used to triage patients over the telephone to determine any problems encountered by patients.

Works towards independently requesting investigations such as X-ray, Ultrasound, and MRI in accordance with trust protocol. Is able to give patient information and advice in accordance with local guidance. Communicated with directorate management team to ensure timely patient care and appropriate use of resources.

Audit the progress of VFC with attention to detail and accuracy and produce reports as requested and present at Directorate governance meetings.


Ensure accurate documentation of information given to patients within notes To ensure patient enquires are promptly answered and issues are communicated with the on call orthopaedic consultant Facilitates good communications with all agencies involved in the continuity of patient care.

Acts as a resource for patients, relatives and carers requiring information and advice regarding episode of care. Arrange follow ups for patients identified in the treatment plan and ensure these are communicated with the patients.
