Description

To work as a member of the Community Health Team, providing rehabilitation, nursing care, and Therapy to patients in their homes, community care settings and/or in a group setting.

The Community Health Team (CHT) will manage via a single locality/Network caseload all patients along a step up/down model which will incorporate care from a two hour response, short interventions, rehabilitation, through to the management of Long term conditions and end of life care.


The aim of the team is to maximise the independence, health, and well-being of adults in order to enable them to return to independent or supported living in the community wherever possible.


  • To communicate effectively and in a professional manner at all times with key stakeholders, members of CHT, patients and their carers or advocates to promote seamless care. Adhere to Caldicott guidance and maintain standards of confidentiality
  • To have good communication skills, utilising tact, empathy, compassion and reassurance including communication with people who are dying, those who experience mental health issues, cognitive or sensory impairment, learning difficulties and people whose first language is not English and require interpreters. Be able to gain informed consent prior to treatment of patients and be able to work with patients or carers to overcome barriers to care.
  • To encourage patients to maintain optimum independence through participating in all decision making related to their care. This will involve talking to them about their needs and preferences in relation to activities such as eating and drinking, meals, washing and dressing, sleeping, and taking part in individual, and group therapy activities.
  • To resolve any queries or problems that are raised by patients or their carers as they arise wherever possible or to ensure their key worker or a senior member of the team is informed to resolve the concerns.
  • Demonstrate customer care skills when listening to concerns, dealing courteously with visitors, staff or patients at all times, including during telephone conversations.
  • To attend meetings as delegated


The CHT works with adults in the community who have various needs such as long term neurological conditions, recent surgery, risk of falls, palliative care, wound care needs, reduced physical and functional ability.

CHT is a 7 day service operating 8am -8pm.

Patient Care

  • Together with clinical colleagues, deliver care to meet the assessed needs of patients and clients.
  • Cooperate with clinical colleagues to understand their roles within the team and appreciate patient priorities.
  • Report any clinical concerns or abnormalities to a registered professional.
  • Deliver care which always upholds patients' privacy and dignity.
  • Deliver care to all patients' regardless of their values, customs, gender, race, culture, religion or preferences.
  • Participate in risk assessment and baseline admission of patients to the clinical caseload, including assist in initial and ongoing risk assessments that will ensure that the patient's home remains safe for rehabilitation/ care provision. This involves reporting to the multidisciplinary team any suspected or obvious risk to patient or staff.
  • Undertake monitoring of vital signs and record these accurately, giving a simple explanation about their purpose and relevance. Demonstrate competence in undertaking these observations. E.g. Blood pressure monitoring, blood glucose monitoring, urinalysis, temperature, pulse, weight, oxygen saturation and respirations.
  • Undertake Phlebotomy following training and assessment of competency.
  • Monitor patients' nutrition and fluid intake, undertaking risk assessment and report any concerns.
  • Assist patients with hygiene needs as part of the palliative care pathway.
  • Assist in assessment of patients continence needs. Undertake stoma care, catheter care and catheterization after training and competency assessment.
  • Assist with turning, moving and repositioning patients using manual handling aids. Ordering pressure relieving equipment and monitoring pressure areas, acting according to any concerns and reporting to a senior colleague.
  • Undertake basic wound care following training and assessment of competency.
  • Under direction of the Clinical / Team lead participate in training to develop competence specific to the clinical area.
  • Assist with the administration of medications according to local policy and following competency assessment. E.g. Eye drops, suppositories, oral medications via tablet aid and ear drops.
  • To be able to recognise improvements or adverse changes in the patient's condition, and report these immediately to a qualified Nurse or Therapist, documenting all observations and actions clearly.
  • Collect clinical specimens on the request of registered staff and send to the laboratory via Courier.
Receive and check and unpack deliveries to the service and report on stock levels.
