Cyfle Cymru is a Peer Mentoring project which helps people develop confidence to make positive life changes, and provides support to access training, education, volunteering and employment.

Our peer mentors draw on their own lived experience to support people affected by substance misuse and/or mental health conditions.

Cyfle Cymru will support people aged 16 to 24 (Not in education, employment or training) and those 25+ that are economically inactive or long term unemployed.


Job Purpose:


  • To ensure all project outcomes are delivered on target
  • To manage the Peer Mentors and Volunteers
  • To deliver Peer Mentor services as follows:
  • Provide support and supervision to a team of peer mentors and volunteers
  • Coach and mentor, the Peer Mentors
  • Conduct caseload reviews
  • Support the Peer Mentors in raising awareness of the service
  • Work alongside the team structure in delivering the service and outcomes for the scheme
  • To support the Project manager and referral agents in raising awareness of the service

Responsibilities:


  • To play an active part in the Cyfle Cymru project, developing personal and professional skills and experience in effectively supervising and mentoring others
  • Motivated the team to achieve the project outcomes
  • To ensure all project outcomes are delivered on target
  • Develop and implement a timeline to achieve targets
  • Conduct monthly performance reviews including caseload reviews
  • To lead/chair relevant meetings with peer mentors
  • To develop and deliver effective ways of supporting and supervising paid and volunteer peer mentors in the delivery of a highquality service, with guidance from the Cyfle Cymru area contact.
  • To oversee the provision of peer led support, information, guidance, advocacy and other interventions to participants of the peer mentoring service, based on their assessed needs, as part of a package of care and progression
  • To allocate participants to appropriate peer mentors based on location, risk, previous association, knowledge and skills
  • To liaise with other organisations and to advocate, when appropriate, on behalf of service users and peer mentors
  • To hold a caseload of people requiring mentoring through treatment and into meaningful activities
  • To monitor all activities and ensure that they are recorded accurately and within agreed timescales in accordance with Welsh Government requirements
  • To represent the Out of Work service at meetings and events adhering to a professional code of conduct at all times

Policy and service development:


  • To adhere to agreed Health and Safety and Security procedures/policies.
  • To adhered to Safeguarding policies and to ensure this is recorded effectively.
  • To identify and contribute towards any aspects of the peer mentoring service where improvement can be made and to advise service and managers accordingly.
  • To assist with the delivery of peer mentoring training if required.
  • To attend regular meetings with the project manager

Professional:


  • To participate in the continuous learning within the Out of Work Service academy and Adferiad appraisal process.
  • To operate within and observe partnership policies and procedures as amended and updated.
  • To make regular use of supervision.
  • To be responsible for working within own level of competence.
  • To undertake internal and external training as appropriate, and as required as part of Continuous Professional Development.
  • To develop and maintain professional working relationships with external agencies.

£25,490.00 per year
