The Role:

  • Joining our Learning Disabilities team, supported by experienced colleagues and qualified social/health care professionals, you'll use a strengths-based approach to assess an individual's needs and provide them with support and guidance to maximise their wellbeing. Strengths-based practice focuses on people's resourcefulness, strengths and abilities to enable them to develop the best strategies to face life's challenges and live independently and safely in the community. Using your effective relationship management and negotiation skills, you'll collaborate with service users, their families, providers and other health care professionals.

    Travel Requirements
  • You will be required to travel to various locations within Hampshire or other local authority and it is essential that you have a current drivers' licence and access to a reliable vehicle on a regular basis.
