JOB SUMMARY

To work under the direction of the registered nurse providing high quality person centred care to patients undergoing assessment for a suspected and/or confirmed Haematological Cancer diagnosis.

The role involves supporting care delivery, coordinating patients' pathways, evaluating patient/carer needs, and providing information and support to those living with Haematological cancer.

To be a part of the Haematology cancer MDT, ensuring effective communication across different healthcare providers.

Responsibilities:

  • Implement plans of care with patients and contribute to care plan modifications.
  • Provide advice to patients, relatives, or healthcare professionals.
  • Contribute to service development, protocols, and policies.
  • Maintain stock control.
  • Update patient records and perform data processing.
  • Participate in audits within the area of care.
  • Adhere to Trust standard operating procedures and policies.

Qualified staff will be available for advice.

General clinical duties:

  • Contribute to holistic needs assessment and individual care plan development under a registered nurse's guidance.
  • Implement, monitor, and review care plans with patients and carers.
  • Evaluate care outcomes with the registered nurse.
  • Review patients on the ward under a registered nurse's supervision.
  • Recognize and respond to emergencies, alerting the team appropriately.
  • Answer incoming calls, triage patients, and provide initial assistance.
  • Provide telephone advice and support, directing patients/carers to suitable resources.
  • Organize appointments and investigations efficiently.
  • Liaise with the MDT coordinator for seamless patient care.
  • Identify patient needs and provide informational support.
