An exciting opportunity has arisen within Primary Care as a Care Coordinator at Balham, Tooting & Furzedown Primary Care Network (PCN).

You will need a caring and compassionate nature, excellent organisational skills and enjoy working with people.

The world of General Practice is busy and ever changing, so the ability to learn quickly and be adaptable to rapid change is essential.

The role will require an ability to organise and prioritise your own workload, therefore you should be comfortable working independently and as a committed member of a multi-disciplinary team.

You will work closely with GPs and other primary care colleagues within the PCN to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers (if appropriate), ensuring that their changing needs are addressed.

Salary:
£26,000.00-£29,250.00 per year
