Job description
Oaklands Support Limited have several exciting opportunities for Support Workers, to join our expanding team in Derby. Successful applicants will work closely with Team Leaders to effectively develop our supported living projects.

Successful applicants will support customers with Mental Health and Learning Disabilities gain skills, confidence and plan for a successful future.
As a Support worker you will be offering positive opportunities to our customers for them to focus on their strengths and develop their abilities to achieve their hopes and wishes for the future.
You will work alongside our customers to work towards these goals through a personalised support plan which breaks their identified goals into steps that are easier to achieve.
Key responsibilities are as following:
- Ensure a highquality service is delivered to customers at all times
- Assist with writing support plans and risk assessments
- Develop and maintain positive working relationships with our customers and their families within the community, while working and engaging with other relevant professionals, agencies and health workers
- Support customers in daily living skills
- Organise and coordinate customers with activities and appointments
- Support with their health, wellbeing and medications
