We are recruiting 3x Band 7 Clinical Experts for the Lambeth Focussed Support Services.We have 1 vacancy at each Living Well Centre, the Lambeth South-East and North Living Well Centres which are based at 312 and 332 Brixton Road, and the South-West Living Well Centre based at Gracefield Gardens in Streatham.
Focussed Support service offer medium to longer term mental health care to residents in the Borough of Lambeth aged 18 and over regardless of diagnosis using the Care Programme Approach.
The Lambeth Living Well Network (LWN) Alliance brings together Certitude, Lambeth Clinical Commissioning Group, Lambeth Council, South London & Maudsley NHS Foundation Trust and Thames Reach organisations to deliver an innovative way of providing mental health support for those who need it in the Lambeth Borough; a fully integrated and coordinated mental health system which focuses on people's strengths and supports their whole health and wellbeing.
As part of our journey, to realising a fully integrated system, we have set up 3 Living Well Centres in Lambeth.
You will be expected to work with the service lead, team leader and other senior members of the team to provide management responsibility for the delivery of care within an integrated community team, in accordance with trust policies and procedures.
You will co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, and other services within the Trust and to ensure effective interface and continuity of care between disciplines and services.
The post holder will take a lead in the development of identified areas of professional practice within the service to ensure high levels of clinical excellence and that training and professional development needs are met within the team.
As a senior member of the team, you will take specific responsibility for the provision of regular supervision and appraisal, of identified team members, ensuring the delivery of high-quality evidence-based practice, within the overall structure of the team.
You will also provide care coordinating for a small caseload of complex cases using evidence based/client centered principles to assess, plan, implement and evaluate interventions.

Key responsibilities:
- Management of Care and Practice
- To act as Care Coordinator for a small case of complex cases using evidence based/client
- Ensure that all clinical practice is developed using up to date evidence base, ensuring that all staff have the appropriate skill base and supporting mechanisms to access training to work with the service user group.
- Ensure systems are in place to support user and carercentred involvement in the planning and provision of care.
- Ensure that allocated staff receive regular supervision and appraisal and are offered essential and developmental training in accordance with their personal development plans, and the needs of the service.
- Ensure that all service users are made aware of, encouraged, and assisted to use all services available to them.
- Ensure that the requirements of all mental health legislation are met with particular regard to the Mental Health Act 1983 (including amendments and Code of Practice) and Community Care Legislation (eg. CPA, Supervision Registers and Supervised Discharge etc), NSF, Child
