Proline Care provide high quality support staff enabling people to live within their own homes, making their own choices, in their own community.

We are a well-established domiciliary care agency, based in Birmingham City Centre.


The role of Care Coordinator is a key role in the company that will require the individual to work alongside the senior Managers to ensure the smooth running of visits to our clients.

For the Care Coordinator role, the successful applicant will have had at least one year's previous experience in a care setting

  • This is a fulltime role, working 35 hours per week with additional paid on call duties.

The Care Coordinator's duties will include:

  • Accepting care packages and assigning care workers to service users in an appropriate manner
  • Producing care workers' rota and managing the roster software with Care Planner (training provided)
  • Using electronic monitoring software to monitor care workers' attendance and timeliness for visits
  • Arranging care reviews and risk assessments for service users with the Care Management Team
  • Conducting telephone monitoring and ensuring service quality
  • Speaking with services users and families over the phone.
  • Be a part of the ON-CALL Service.
  • Maintaining high quality standards, systems and procedures set by the company and in line with CQC Compliance and Health & Social Care Act 2008 and contract compliance as applicable.
  • Taking instructions from the Homecare Manager and Directors

Care Coordinator Requirements

  • Recent experience within a care setting is required for this role.
  • You need to have good computer skills.
  • Preferably have worked with rostering systems (Care Planner)
  • Be able to problemsolve
  • Have an excellent customer serving manner
  • Excellent communication and organisational skills
  • Selfmotivated and proactive.
References and DBS enhanced check is required for all of our roles.


Salary:
£22,000.00-£30,000.00 per year
