Job overview:

An exciting opportunity to join our newly formed Home First therapy team.

We are a rehabilitation and reablement service covering the whole of Salford facilitating clients discharge back into their own home.

Our vision for Home First is 'People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary'

We are looking to expand our current team looking for Band 3 Therapy Assistants to join our service.

This exciting and rewarding role supports people to be discharged from a hospital setting by providing reablement in their own homes.

You will receive referrals from Therapy Leads and you will be working along side a fully Multi-disciplinary Team within the Community Services to carry out the role.

Good communication skills are required in order to arrange appointments with patients and their relatives, along with explanations on how to use the equipment.


Working with the guidance of the Therapy team you will help reable people who have been discharged from a hospital setting and help them to remain in their own home setting to enable them to be independent.

You will record your work on the electronic patient record system and communicate with the Therapists regarding the outcome of the people in their surroundings.

Full training and supervision will be given.


The Service operates over 7 days, currently covering the hours of 8am to 6pm. (weekends occurring 1:4)


Essential Criteria:
MUST HOLD A FULL DRIVING LICENCE AND HAVE USE OF THEIR OWN VEHICLE


Main duties of the job:


The Salford Rehab & Reablement Team provides a life changing service to the people of Salford by helping them to regain or attain as much independence as they can in their own home.

Our Home First Therapy Assistants do this by motivating and inspiring people to re-learn daily skills and enhance their quality of life.


You will be fully trained to assist people to meet their care needs, which can range from motivational support to personal care and promptly alert the team where there are any changes in the person's health and wellbeing or any possible risks.


  • To complete equipment referrals from Occupational Therapists to deliver and fit therapy
- equipment, grab rails and stair rails in patients home

  • To document all patient related information / job completion of referrals on the Electronic Patient Record
  • To feedback verbally to the referring Occupational Therapist the outcome of the equipment
  • To liaise with equipment services at Burrows House to ensure the correct therapy
- equipment is ordered and

  • To carry out any appropriate administrative tasks within the
  • To contribute to Occupational Therapy team working such as reviewing patients use of
- equipment that has been provided by Occupational Therapists during follow-up

  • To support other AHP services such as Podiatry / Orthotics with equipment provision
  • Assistant Practitioner Training Opportunit

Due to the role working in the community a Full Driving Licence and the use of your own vehicle is essential. Flexible shift patterns and various hours available.


The Home First Reablement Service operates 7 days a week from 8am to 6pm and various shift working is available.

The successful applicants will work on a rota that is provided in advance to cover shifts that includes weekends and Bank Holidays.

Detailed job description and main responsibilities:
