The Role


This opportunity is based in Fen House, Ely which provides rehabilitation to up to 25 individuals with an acquired brain injury.

It is a unique community where the people we support can address their cognitive, behavioural, physical, and emotional difficulties.

You will work alongside occupational therapists, physiotherapists, psychologists and speech and language therapists who work collaboratively with support workers to enable individuals to achieve their goals, such as regaining the ability to walk, talk or re-learn daily skills.


To provide a high quality confidential and comprehensive support service to the Service Manager and Clinical Team at Fen House to ensure efficiency in the service users' care.

This will involve focusing primarily on ensuring an efficient referrals, admissions and discharge process for service users.


MAIN TASKS AND RESPONSIBILITIES

  • Gather referrals information from the clinical coordinator and review with the clinical team to determine appropriate referrals for assessment and rehabilitation
  • Visit local hospitals and hospitals in neighbouring counties to undertake preadmission screening assessments, gather information to feedback to the service manager and clinical team for effective decision making
  • Communicate with hospital patients about the service that Fen House can provide; be an ambassador for Fen House
  • Compile preadmission assessment reports to commissioners within 48hrs of assessment
  • Attend weekly meetings to discuss referrals and admissions with the team
  • To assess social care needs on client admission and communicate with the clinical key worker
  • Provide information, directing and prioritising queries as appropriate to ensure efficiency and effectiveness of service delivery.
  • Be responsible for incoming referrals and input appropriately onto the correct databases. This will include maintaining the CRM and ensuring all entries are completed in a timely and accurate manner.
  • Attend service user discharge review meeting and make links with external agencies to secure future move on placements for service users
  • Ensure all welcome information is available to new service users and family members on arrival
  • Regularly maintain the service users' discharge care plans/profiles detailing discharge plan/destination and progress to date.
  • Assist with the discharge process by ensuring the discharge check list is reviewed, that the service user's own GP is updated ap and any other relevant information is passed onto the appropriate external professionals
  • As part of the discharge process invite former service users back into the service after 6 months and with the support of the Assistant Psychologist complete the outcome measures report
  • Use clinical resources to provide high quality, best informed, assessment and discharge reports for the clinical team and manager.
  • Ensure all Trust policies and procedures are adhered to in the course of the day to day work activities, Health & Safety procedures are adhered to, thus providing a safe environment for residents, staff and visitors.

About you

  • Educated to degree level or equivalent in relevant field ie Psychology, OT Health & Social Care, NVQ Level 4 or proven relevant experience in the same or similar role.
  • Experience in working in health and social care field.
  • Current Clean Driving License.
  • Proven track record of planning and organising own work schedule and adhering to strict deadlines.
  • Proven experience of building and maintaining strong relationships with other agencies and professionals.
  • Experience dealing with Funders and Commissioners with enquiries and referrals.
  • Proven experience of working in a care setting.
  • Experience working with a high level of confidentiality and discretion in all matters.
