The Living Well Team is committed to providing our residents with vibrant and meaningful activities both within the care home and the community, helping them to lead fulfilling lives
As an outstanding rated care home, we are looking for someone with the WOW Factor
Ideally, you will have a proven track record of working with and understanding the needs of older/ vulnerable adults and have been involved with the development and delivery of activity both in groups and on a 1:1 basis.
To be successful in this role you will:

- Have a natural ability to build relationships with people from all walks of life with varying physical and mental abilities
- Be forward thinking with the ability to plan and organise time effectively
- Be an excellent communicator and coordinator who inspires teamworking and sharing of information
- Be comfortable with technology and digital systems including using a Microsoft software
- Need to be flexible to working alternate weekends and some evenings
As a Living Well Co-ordinator you will:
- Inspire and motivate residents to take part in meaningful engagements
- Spend individual time with residents, relatives and team mates to deliver personalised activity programmes, assessing peoples' needs proactively and relaying information back as appropriate to the Living Well Team Leader and Care Managers
- Deliver a wide variety of activity across a range of interests such as Music, Exercise, Trivia, Arts & Crafts and many more
- Assist in the development of community links with local groups and help arrange trips and excursions
- Support care teams over mealtimes to ensure this is a highlight of a resident's day
- Understand the need to promote the philosophy of privacy, dignity, independence, choice, rights and fulfilment for all residents.
- Participate in staff and resident meetings, as and when required, to offer and receive feedback on activity
- Complete daily digital documentation in a timely manner to evidence resident engagements and support in the evaluation of activity
