Job overview:


An exciting opportunity has arisen for an individual with a keen interest in falls prevention to join a pilot run by the Inpatient Falls Team.

This role will focus on providing support to the inpatient teams with falls risk assessment for patients who had multiple falls and support with advice on most appropriate fall prevention strategies for those patients to decrease the likelihood of inpatient falls occurring.

We are looking for an experienced, caring, motivated Assistant Practitioner to join our developing Inpatient Falls Team. You should have experience of liaising with different healthcare professionals and innovative delivery of excellent standards of care.


You will need to be able to demonstrate a real enthusiasm for falls prevention agenda as well as knowledge of the current standards and recommendations in this area.

You should be able to work autonomously with mínimal supervision and be able to prioritise your caseload.

You will need to have the ability to travel independently across the county to fulfil this post's requirements.


Main duties of the job:


To work alongside the inpatient ward teams within the ACS and MH setting as part of the Trust's Inpatient Falls Team supporting multi-factorial assessments of patients referred to the service with aim to decrease the likelihood of multiple during patient's admission.

The post holder will work under structured guidelines from Clinical Specialist Falls Lead and with mínimal direct supervision providing falls prevention advice to patients and members of the ward team
in accordance with agreed protocols.


One of the main aspects of this post will be providing support the inpatient ward teams with learning from inpatient fall incident by utilising post fall huddle and reflexion.

They will carry out a review of multifactorial falls risk assessment and personalised care planning and communicate and support the onward referrals to internal and external services / agencies.

Part of this role will be to triage referrals received from the incident system and directly from the ward teams, keep accurate documentation of input provided and raise any issues or emerging themes or
trends to the Clinical Specialist Falls Lead.
