Description

As a Senior Primary Care Mental Health practitioner, the post holder will work as part of a dynamic team in delivering an effective service supporting Primary Care Networks (PCNs) in Wolverhampton, Sandwell, Walsall and Dudley.

The primary focus of the post holder will be to support high quality service delivery of the Primary Care Mental Health Service across a range of practices within their designated PCN and lead on the co-ordination of the PCNs SMI registers.

The post holder will be required to engage a range of key partners and stakeholders and lead on joint working across health and social care.

In particular, they will need to work closely with colleagues within Black Country healthcare Mental Health Services, the PCNs, other provider organisations, local authority/Office of Public Health, and the voluntary and community sector.

They will also work alongside the Primary Care Networks to support them in meeting their priorities around prevention, reducing health inequalities and population health management.
