Key Clinical Responsibilities To provide a lead role in supporting the practice in triaging mental health concerns raised by on the day patient requests, providing specialist advice and support to the patients of the surgery To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialised advice concerning care when appropriate and liaising with other practice staff such as GPs, Pharmacist, Social Prescribers.

To offer brief interventions as needed eg problem solving, decision making, building resilience, building on personal strengths, risk de-escalation, medication advice and prescribing if a registered NMP Working as a member of the primary Care MDT and taking responsibility for the development and implementation of specialist mental health support within the team To provide a navigation role to help clinicans make appropriate referrals to the relevant community and secondary care services To be confident working as the dedicated mental health practitioner, offering support and signposting as appropriate.

To receive and communicate sensitive information to clients in situations which may be highly emotive, and where barriers to acceptance and understanding may need to be overcome, requiring developed communication and empathic skills.

To carry, manage and be responsible for own workload and the quality of care they deliver within the clinical area that he or she is responsible for, liaising regularly with GPs, practice staff and other relevant organisations, and undertake administrative tasks related to patient care where necessary.

To maintain and update patient records on the computer in accordance with practice policies and professional guidelines e.g. NICE and other evidence-based guidelines.

To liaise with appropriate resources when necessary To ensure effective lines of communication are maintained to promote good working practices and ensure that service user information is passed on appropriately to other services where required.

Facilitate mental health and strengths - based assessments in Primary Care, and support the Primary Care Team in accessing appropriate level of services.

To participate in professional meetings as necessary where there is notification of serious concern i.e. Safeguarding and risk management concerns or in multi-disciplinary reviews. To keep accurate, contemporaneous electronic and written documentation on Systmone, care plans and reports, and outcome measurement in the agreed systems and in designated formats and within agreed timeframes and to be responsible for maintaining a high standard of clinical record keeping To work to the local GP practice code of conduct
