Key Responsibilities Be a key link for the patient. Offering a supportive conversation, an e- HNA & Care Plan working with a defined cohort of GP practices within the Primary Care Network, to support those living with cancer.
To manage a caseload and respond to client referrals in a timely manner as per project model Be responsible for the administration of the service on a daily basis: running weekly EMIS searches for newly diagnosed cancer patients, contacting patient to arrange appointments, data collection and submission, filing, general admin etc.
Be responsible for the smooth pathway for the patient and community providers brokering access to a wide range of local services including physical activity, dietary, emotional, therapeutic, Macmillan Information and Support centres and benefits to support the individual through treatment and recovery and deliver desired outcomes Empower individuals to have more confidence in dealing with their treatment and enable them to understand the lifestyle changes they could make to improve their overall health and wellbeing Be responsible for direct clinical data inputting onto the EMIS system for the HNA assessment and care plan generation; Ensuring compliance with information governance.
Facilitate proactive communication with patients to engage with Health & Wellbeing, including proactive referrals to physical activity services Attend mandatory training, training on the undertaking, delivering and reviewing Holistic Needs Assessments and Care Plans for those affected by cancer in line with the national programme; and undertake personal and professional development linked to the role and Macmillan requirements Contribute to the evaluation of the service, collate and input timely data and suggest/implement service improvements Keeping up-to-date with National/Local Cancer Strategies To be accountable for ensuring that all clinical records, including work diary, are maintained in accordance with the practices records management and Information Governance policies To be accountable for ensuring personal compliance with all General Practices policies and procedures including safeguarding children and adults and health and safety, alerting the line manager of any issues or concerns in relation to delivery of the service To produce performance and quality improvement reports as required by Operations Manager Work with the practices by providing an advocacy role of support to those identified as needing support who are referred for a 2 week referral Scope community assets and develop a service directory for the PCN area Work collaboratively with the current Social Prescribers and Health and Wellbeing Coaches, through peer support and as part of the wider Team Work proactively with Acute Trusts to understand the discharge process and be able to positively input into processes that optimise the patients journey Undertake other duties as may be reasonably required within the general terms of the job description For full Job Description please see attachedMore jobs from MIAA Solutions
