Intensive Support Worker (Complex Lives) - Belfast - Outreach
£23,218.65 per annum, pro rata

35 hours per week

Fixed Term - 31st March 2024 (with possible extension depending on funding)

Your new role

As part of a new initiative, The Complex Lives project, you'll ensure effective case management for a caseload of approx.

7-10 highly complex and entrenched individuals who experience multiple connected and dependent challenges, usually including homeless/unstable housing, problematic drug use, mental ill health, offending behaviour, poor physical health, often underpinned by trauma.


The primary role of the post holder will be to act as a trusted key worker and to ensure effective coordination of a wraparound support plan for each client which will have been developed through the Belfast Complex Lives Multi Disciplinary Team (MDT) process.

Essential Criteria

  • A formal Level 3 qualification in a social care field, e.g. diploma in health and social care or equivalent
And

  • At least 3 years paid relevant work experience; with knowledge of managing complex individual cases and coordination of work with people affected by interdependent issues such as homelessness, substance misuse, offending behaviour, domestic and sexual violence
  • Ability to work on a rota basis (7 day week
  • Mornings, Evenings & Weekends)
  • A full driving licence and full access to a car insured for business purposes
  • Willingness to undertake regular travel in the local area
