Job Purpose


The post holder is employed by Somerset Care Limited, working within a defined geographical area planning and organising the work of the Community based staff, in the most effective manner, whilst maintaining continuity of care for Customers.

This role is to provide the highest standard of care provision that is compliant to the standards set by the Care Quality Commission, working "with" Customers to achieve outcomes, rather than working "for" them.


Key Tasks

  • To carry out day-to-day planning of rotas across the geographical area served by the base.
  • To make continuous improvements to rotas to increase their effectiveness e.g. by reducing travel times between care calls and providing continuity of Community based staff to Customers as per the base targets.
  • To work with the Community Team Supervisors for your area to ensure you coordinate Community based staff and services efficiently and effectively, and liaise with staff accordingly e.g. staff absences and annual leave.
  • To ensure that all new referrals are acted upon within agreed timescales.
  • To allocate all visits to enable rotas to be sent out each week within a specific deadline.
  • To effectively manage availability to maximise capacity requirements.
  • To minimise the volume of unallocated regular bookings to within the agreed base target and to ensure all work handed back is reallocated.
  • To maintain accurate and up to date information on the IT system in respect of live Customer and staff data, including all changes to Customer and staff information, as well as ensuring all relevant people including Customers are updated.
  • To develop good relationships with Community based staff, liaise with Customers, and Partners/Stakeholders/Commissioners in a professional manner, and to deliver excellent customer service.

General

  • The Company Induction will give you the skills and knowledge to understand your role.
  • You will attend regular review meetings, annual appraisals, team meetings, training courses and followup refresher courses as required.
  • To participate in regular supervision, and annual appraisal meetings.
  • Undertake other such duties as may be required by the Manager or Supervisor.
  • You will maintain a professional and hygienic appearance.
  • You will work in line with the policy on Infection Prevention and Control, according to your role.
