Job Purpose To work within the Integrated Neighborhood team to provide assessment and input to people within the Nottingham City Community.

To work holistically in providing assessment and treatment of health and social care needs, working across professional boundaries and within legislation, policies and procedures and core values.

Dimensions You will be working in a variety of community settings which may include a clients own home/care home and out-patient clinics setting up assessment and rehabilitation programmes for clients carers and families.

To participate actively in the services training strategy, including mandatory training and otheridentified necessary for this role.


Key Responsibilities Clinical Work in accordance with the code of conduct for Health Care Support Workers and Adult Social Care Workers in England Skills for Care & Skills for Health 2013 To accept responsibility for a designated workload, taking an active role in promoting independence and meeting the needs of the individual.

To undertake assessments, care planning and interventions with individuals, under the supervision of qualified staff To review and adapt individual care plans as appropriate.

To work in accordance with competencies, recognise own limitations and the need to seek support from other members of the team.

To refer on to other services when the individuals needs are beyond the assistant practitioner role.


To promote health and wellbeing when working with individuals and their family/carers Managerial and Leadership To provide support to other Assistant Practitioners, Support Workers and contribute to the development of students as appropriate.

Education and Development To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames.

To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with organisational policy.

Also, to evidence that you proactively engage in continuing professional development activities.

Clinical Governance To maintain comprehensive and accurate records in line with the organisations policies. To maintain a safe working environment and comply with health and safety regulations, policies and procedures. To comply with information governance requirements and maintain confidentiality at all times. To actively promote equality and diversity at all times, in line with the organisations policies.

The post holder will comply with current legislation, the organisations policies, procedures, values and vision.
