An exciting opportunity has arisen within the Nursing Governance Team for a Serious Incident (SI) Lead.


As a Serious Incident (SI) Lead, you will be responsible for undertaking Serious Incident reviews - overseeing the whole reporting of the investigation / review, and recommendations and learning in addition to ensuring that any practice changes that are required are carried out according to the recommendation contained.

The principles of openness and transparency will underpin every step and the postholder will be responsible for ensuring the spirit behind the Duty of Candour is implemented as well as at a policy level.


The role holder will ensure that the Trust quickly responds to and learns from incidents and complaints and alerts in addition to being responsible for liaising with Adult and Child Safeguarding specialists to ensure learning from serious cases reviews and analysing incident data highlighting trends and areas for action whilst working alongside Executive Directors, Clinical and Service Directors, for investigations of extreme complexity and sensitivity.


This role operates across a number of Care Delivery Services within a defined geographical area (although some degree of flexibility will ensure resilience and emerging specialism).

The postholder is responsible for investigating serious incidents with the services and families, and embedding learning and service improvement.
