At Thrive Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that make people happier and healthier We're looking for an Healthy Lifestyle Practitioner to join our One You Westminster and One You Kensington and Chelsea Service.


The One You Healthy Lifestyle Practitioner plays a vital role in helping people to improve their short and long term health prospects.

The main focus of the role will be delivering two different healthy lifestyle courses, weight management and physical activity.

The post holder will work with closely with clients to educate, inform and empower them to take control of their own health, thus achieving, or starting the process of working towards any health based goal they may have.

The Practitioner will monitor client progress and communicate this with their line manager and individual client Health Coach Advisors. The position will also include the important tracking of client progress data using our service database and spreadsheets.

A typical day as a
Healthy Lifestyle Practitioner might include:

  • Delivering evidence-based interventions that focus on nutrition, lifestyle, physical activity and behaviour change. Interventions will be delivered to groups in a hybrid style delivery, including mostly face to face work and some online sessions.
  • To work with Health Coach Advisors to ensure clients are supported to make behaviour changes through their various interventions with the service.
  • To implement aspects of an individual's health improvement plan including SMART goal setting/taking into account their risk factors, individual lifestyles and motivations.
  • Working collaboratively with the team and partners to create referral routes and meet targets.
  • Ensuring an excellent and safe experience for residents using our policies and procedures.
  • Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments we're always looking to share knowledge and resources
  • Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values
  • Evening work will be required at least twice per week (Monday to Thursday), for which you can take time back at another point in the week.

Contract:

Permanent, Full Time (37.5 hours)


Salary:

Starting at £24,000 per year (rises to £27,000_)_
