An exciting opportunity has arisen to join County Durham and Darlington NHS Foundation Trust and to work across the Durham and Darlington place based systems.

This builds on the work underway to address healthcare inequalities and to ensure the unjust differences in health and care access, experience and outcomes across the population are addressed.

This is a key role for the place based systems and will provide population healthcare leadership as well as support the Durham and Darlington system in their ambitious plans to improve health of the population, tackle health inequalities, ensure efficient use of resources as well as the NHS contribution to addressing social determinants of health and economic regeneration.

The Trust executive and senior leadership teams as well as the board have received a number of reports on the role of the organisation in population health and tackling health inequalities providing a very good foundation for this role to further embed this approach across the organisation and across the system.

This role will play a key part in supporting integration of prevention, health and care across the two pace based systems
